Required Documents
To be
considered for enrollment in The Child School/Legacy High School, a parent/
guardian must mail the following materials:
- Applicant’s Individualized
Education Plan (IEP)
- Neuropsychological or
Psycho-Educational Evaluation
- Evaluation from Current
Teacher, and/or Resource Room Teacher, as well as an evaluation from his/her Home
Room, English, and/or Math Teachers.
- Copy of Last Report Card
- High School Transcript
- $50.00 application fee
Evaluations from anyone that the student sees on a
professional basis, both in school and outside of school. Please submit evaluations from those professionals listed below that the student has been seeing on an ongoing basis:
- Play Therapist
- Tutor
- Speech and Language Therapist
- Occupational Therapist
- Social Worker
- Counselor
- Psychologist
- Psychiatrist
- Any additional records you
feel might help us better understand your child's needs.
**Important** Do not
send originals as we cannot return any documents. Faxed packets are NOT accepted. Mail only when the packet is
